Insurance

We accept most of the commonly available forms of managed care (HMO, PPO), private indemnity (classic, private insurance) and Medicaid, subsidized insurances. We do this as a courtesy to our patients so they may have as a flexible a choice in their health care decision as possible.

However, due to the variety of insurances, with their individual requirements of co-payments and referral networks, we must insist that you keep a current valid insurance card with you when you come to the office for your visits. Inaccurate or out of date insurance information prevents us from receiving payment for the services we have rendered to you, therefore we will ask for your insurance card at each visit. Also, please be prepared to pay your co-payment at the time of your visit.

For our managed care and Medicaid patients, we would like to remind you that a referral from your primary care physician is required before subspecialty care can be given. We request that whenever possible, you coordinate your subspecialty care with your regular physician and that you give us at least a week to arrange for your consultation or follow-up care. In case of an emergency, our Referral Coordinator and your primary or covering physician will arrange for appropriate care on an ASAP basis.

These conditions make it necessary for us to have proof of valid insurance before your child can be seen. In the event that no proof of current insurability is available, we must ask that payment for services be made in full at the time of your visit. Please see our financial policy.
