Well Visit Forms,Camp Forms, School Forms, etc.

Due to the overwhelming demand of well visit forms, camp forms, school forms, etc, we are requiring a 72 hour window for processing.  Also, the well visit form you are given at your child's physical is your one original copy.  Please make copies not only for your records, but also to disperse to camps, schools, daycares, etc.  Thank you.
