Behavioral/Learning Concerns

You have contacted our office regarding behavioral and/or learning concerns about your child. A common concern for parents and/or teachers is whether a child is having significant difficulty sustaining focus on activities. These attentional difficulties can interfere with performance and behavior at home and at school. If you would like to have your child screened for attentional difficulties, please download and complete the following forms and then make an appointment for us to review them together. So that we may provide the best possible care for your child, please download and forward the following to the office before your appointment:

  • Initial Assessment Checklist. One to be filled out by a parent(s) and one to be filled out by your child’s primary teacher(s).
  • Parent report form
  • In addition, please provide copies of any previous evaluations done at school or by any other health professional.  A copy of your child’s most current report card would also be very helpful.

At your appointment, you will meet with the nurse practitioner to review the assessment forms and potential diagnoses.  After meeting with the nurse practitioner, you may need to follow up appointment at another time with your primary care physician to discuss treatment if indicated.  Please call if you have any questions. Please have all pertinent information frowarded to the office at least 1 week before your appointment. At your appointment, we will review these materials and recommendations for further treatment or evaluation will be discussed.
